New Patients: Beginning the Process
If you would like to discuss beginning psychotherapy, please call me at (312) 527-3807 and leave a voice message. Please leave your phone number and some good times to reach you. During the initial brief phone consultation, we will discuss what you are looking for, answer questions you may have, and decide whether to schedule an initial appointment.
If we schedule an appointment, you can download both The New Patient Information Form and The Psychologist-Patient Services Agreement. Please fill out the New Patient Information Form and carefully read The Psychologist-Patient Services Agreement. If you have any questions, we can address these during our first meeting. Please bring both of these forms to your first appointment.
During the initial first and/or second sessions, we will focus more in depth on the nature of the problem you are wanting help with, and I will share impressions and recommendations. After we have a chance to understand your goals more fully and the nature of the problem, we then develop an individualized plan of how we might work together to achieve the goals you have set for yourself. Therapy can be more focused and short term, or more open ended lasting until you feel you are ready to end. How we proceed depends on the nature of the problem you would like to work on, and your own preferences.
If we discover that I am not the appropriate therapist for you and other resources or professionals may be more helpful in addressing your problem, I will be glad too suggest alternative resources.
I provide therapy on a fee-for-service basis. In addition, I am in-network with Blue Cross/Blue Shield PPO, and some other PPO plans. Payment by cash or check is due at the time of service. I do not accept credit cards.
